Hospice Austin and Austin Palliative Care are now engaging their patients in conversations resulting in POLST documents!

Hospice Austin and Austin Palliative Care are expanding their goals of care conversations with their seriously ill patients to include completion of POLST, portable medical orders, where appropriate. This allows the ongoing treatment plans to embrace patient choices for care they do want as well as care they may not want as their illness progresses.
